Accountable AI: cite the source, then ask before you act

A model that's right 95% of the time sounds great until you remember the other 5% looks exactly as confident. For a brainstorm, fine. For a refill, a tax memo, or a renewal quote, "probably right" isn't a standard you can run a business on.

The fix isn't a better-sounding answer. It's accountability — and it comes down to two habits.

1. Cite the source

An answer with no source is a guess with good grammar. When a Specialist makes a claim, it should show its work: the document, the record, the policy it pulled from. That does two things. It lets a person check the answer in seconds instead of re-doing it. And it kills the most dangerous failure mode in professional work — a confident fabrication no one caught.

Grounding the AI in your knowledge, with citations, is the difference between "the internet says" and "your own files say."

2. Ask before you act

Reading is low-stakes. Acting is not. Sending the email, filing the form, posting the update — those deserve a human in the loop.

So the pattern I build around is simple: the AI drafts, a person signs. Consequential actions wait in a queue — I call it the Mailbox — until someone approves them. You're not trusting the AI. You're approving what it drafted, with the source right there.

This isn't a brake on speed. The slow part of most work is the drafting and the lookup — not the click to approve. Take those off your team's plate and keep the judgment where it belongs.

Why it matters more as AI gets better

The better AI gets, the more I'll hand it — and the more an unsourced, unapproved action can cost. Accountability isn't a feature you bolt on once the model is good enough. It's the thing that lets you say yes to more, sooner, without lying awake about it.
